I was playing and I had a very good ally (he was loyal and true) named Anatolia (the country). We were both doing good (me much better of course) and all the sudden in the little message window in the bottom-left of the screen there are many new messages and suddenly many many dings. It says "Kingdom of so-and-so has arisen from its ashes." Many of those. I look on the political view and my ally (who was once at 15 provinces) is now down to a measly 1!!! It bugged me. Another time, Teutonic Order suffered the same fate, and it popped up saying I lost two territories!! How can I stop it and why does it happen!

If the king dies without a heir, it can happen. I can't think of a way to "stop" it, but you could use the cheat bsswitch and make sure they are married. And about yourself, I don't think you can stop it either, but you can retake the rebeling provence.

stop loyalists and rebels in bordering provinces to conquer towns.
But sometimes even this soesn't help. When some king dies without an heir all provinces that are not adopted start to be either new kingdoms or join their previous owners. (when this happen, you see that kingdom this and that has arisen from it's ashes. See that it emerges always after it's previous owner dies.)

Actually even if you convert all of your provinces, adopt them and all that jazz, there is a chance that when the king dies without an heir (and one of your knights siezes the throne) something like 90% of your provinces divide in single-province countries, all at war with you and between themselves. This can be avoided by simply loading untill it doesen't happen.

Yeah but reloading the game is almost like cheating. I like the challenge of having to conquer all the rebellious provences.

no. ADOPTED provinces stay with your kingdom even in this case

I am not so sure this is true. I was playing as Scotland and had about 50 provinces without an heir to the throne (no prince). I had adopted probably about half of the territories that I had conquered, but when my king died, I had only two of the fifty original provinces. I should have about twenty or so provinces left if adopted provinces stay, right?

Well half of the time my retarded king doesn't have an heir (prince) so one of my knights takes over. This has occured so often but never have I just lost my territories.
