*Most English speaking people use Sumimasen to say I'm sorry, or Thank you or Excuse me when in Japan, because that is what their dictionary tells them. But when a Japanese person uses the phrase it is because someone has done something for them they were unable to do for themselves, like for instance they have no match to light their cigarette and someone lights it for them, they may say "Sumimasen" but their meaning is "Thank you but I am sorry you had to do this for me that I can not compensate you for"*

btw: in fact, the "real" meaning of sumimasen may be what Dobber told..although I know it ina different meaning as "it doesn´t end" (with bothering you)...... you see, its like old english or old german..a very pathetic way to say something, but in general using, the word simply means "excuse me" and nothing more....it is just a very politly way to disturb someone, if you want something....the "phrase meaning" is not what it means in every day language..it more like "its raining cats and dogs" to talk about heavy rain or something like that......e.g., if you want to know the time you might say: "Sumimasen, ima nan-ji desu ka?" ...in japanese the usage of language depends on the speaker and the adressed person...e.g. there are a dozen words for "I", depending on, if you are a man, a woman, if you are talking to a friend,a boss, an older and respectfull person....I guess ohnly really experts understand when which phrase must be used, but there is always a way to be polite and gramatically correct without going to deep into the cultural meaning E.G. "watashi" for "I" is correct and polite and will therefor do the job ;) and counting is one of the strangest things I have ever heard ;) the word for the numbver depends on wether you are counting, persons, things (even a difference between long, edged ones and others) time periods etc...a lot of different words ;)
